If you stop taking Ozempic, your hunger usually comes back gradually. And often some of the weight too. That sounds harsh, but it's fair. The drug works as long as you use it. Why does hunger return?
Your body has some kind of target weight. Hormones, hunger and metabolism work together to maintain that level. In long-term obesity, that level is high. Ozempic helps you fall below it temporarily by making your sense of satiety stronger.
But your body doesn't change that target weight overnight. As long as you take the drug, your hunger remains muted. If you stop, the old signal returns. A hormone that triggers hunger, ghrelin, takes over again. In some people, hunger then feels even stronger for a while than before treatment. This is called a rebound.
This is not a lack of willpower. It is biology. Your body is actively working against permanent weight loss. This is exactly why losing weight and staying lost is so difficult.
How fast does that happen?
An overall picture after your last ozempic injection:

The pace varies from person to person. It depends on your lifestyle, your starting weight and how your body reacts. Semaglutide disappears from your body slowly, in about four to five weeks. Hunger declines at that rate.
How much weight comes back on quitting?
Research shows a clear pattern. People who quit Ozempic without changing their lifestyle, regain on average about two-thirds of the weight they lost within a year. Some even regain their weight completely, often after a year and a half to two years. A recent large analysis from 2026 found an average of around 0.3 kg per month back after quitting.
Note the word average. The numbers say nothing about you personally. Those who have really changed their diet and exercise often retain much more. Those who lean on Ozempic alone are more likely to see a relapse.
Some of your health gains may also return. Improvements in blood pressure and blood sugar often drop back towards previous levels when weight returns. Some benefits, such as lower cholesterol, remain partial.

What can you do about it?
Quitting does not have to mean losing everything. The difference is in the preparation. A few things help:
Phase out by mutual agreement, not abruptly: phasing out often produces a milder setback than stopping all at once.
Get your lifestyle in order before quitting: A steady diet, enough protein and exercise catch some of the rebound.
Consider a maintenance dose: Sometimes a lower dose is enough to maintain your weight without stopping altogether.
Schedule review moments: That way you can see in time if the weight is coming back and make adjustments.
